Challenges in Bone Cancer Treatment
The two main challenges are:
Repairing large bone defects
Eradicating any remaining tumour cells.
Outcomes have not significantly improved for osteosarcoma and Ewing sarcoma, and therefore, there is a need to find better treatments and overcome conventional treatments (surgery, chemotherapy, and radiotherapy.

Follow Up Appointments
Goals of follow-up appointments
To monitor progress and coping with health issues.
To manage long-term toxicity of chemotherapy and radiotherapy.
To manage complications of surgery, e.g. reconstruction
To minimise risks:
There are rare cases of secondary leukaemia, e.g. acute myeloid leukaemia after 2 to 5 years of chemotherapy.
Other risks are germline cancer predisposition syndrome; this can be detected from molecular techniques like whole-genome sequencing (WGS)
Local recurrences are often detectable by patients.
How are these goals achieved?
To perform physical examination of the primary tumours
To conduct periodic cross-sectional imaging to monitor recurrence or metastasis, especially in lungs/lymph nodes. This could be head-to-toe FDG-PET, CT, and/or bone scan.
To perform functional assessment at every visit.
To conduct full blood cell count and other laboratory tests
To follow cancer surveillance programmes especially patients with osteosarcoma and Li-Fraumeni syndrome.
Follow-up Timelines in Accordance with NCCN Guidelines
Generally

Low grade bone sarcoma

Late metastasis, local recurrences and failure of reconstruction
It may occur for more than ten years post-diagnosis in all tumours. There is no fixed stopping point for follow-up.
Chest Surveillance

Returning chordoma (occult local recurrence)
Chordoma is a rare bone cancer that arises in the notochord area beneath the neural tube in the embryological origin. It is a type of mesodermal tissue.


A patient had a recurrence of cancer after the removal of pelvic chondrosarcoma
It is detected by X-rays, the key steps are as follows:
